How to Read Your Child's Psychoeducational Evaluation Report
ByJames PetersonVirtual AuthorThe report arrives as a PDF attachment, twenty-some pages, and you open it on your phone in a parking lot because you have been waiting nine weeks for it. Page four has a table. The table has numbers like 87 and 9 and 68, and none of them are on the same scale, and the eligibility meeting is in ten days.
Psychoeducational reports are written by evaluators for other evaluators. The school psychologist who ran the evaluation assumed the reader already knows that a 9 and a 95 can describe the same average performance. You are the one person at that meeting who knows your child, and you are being handed the document in the language you know least. Learning to read four or five conventions closes most of that gap.
The Three Number Systems in One Report
Almost every psychoeducational report mixes three scoring scales without announcing the switch.
Standard scores have an average of 100 and a standard deviation of 15. Most composite and index scores use this scale: cognitive indexes, achievement scores, adaptive behavior domains. Scores from 85 to 115 fall within one standard deviation of average, which covers roughly two-thirds of all children. A 100 is the exact middle. An 82 is below average but not dramatically so. A 68 sits more than two standard deviations below the mean, which is the range where eligibility conversations usually begin.
Scaled scores have an average of 10 and a standard deviation of 3, and they run from 1 to 19. Individual subtests use this scale. When you see a column of single-digit numbers next to a column of three-digit numbers, you are looking at subtests and composites side by side. A scaled score of 9 and a standard score of 95 describe the same relative position.
T-scores have an average of 50 and a standard deviation of 10, and behavior and executive function rating scales use them. Here the direction reverses. On a cognitive test, higher is stronger. On a rating scale measuring attention problems or emotional control, higher means more difficulty reported. A T-score at or above 65 is typically flagged as clinically elevated, and scores in the low 60s are usually described as mildly elevated. A 68 on the attention scale of a behavior rating form is a finding, not a passing grade.
Percentile Ranks Compress Near the Middle
A percentile rank tells you the percentage of same-age children who scored at or below your child. The 50th percentile is dead average. The 16th percentile corresponds to a standard score of 85, the 25th to a 90, the 84th to a 115.
The uneven spacing matters. Between standard scores of 95 and 105, ten points move a child from the 37th percentile to the 63rd, a swing of 26 percentile points. Between 65 and 75, the same ten points move a child from roughly the 1st percentile to the 5th. Small differences look enormous in the middle of the distribution and nearly invisible at the low end, which is where your child's real gap may live. When a school representative says two scores are "only four points apart," ask where on the scale those four points sit.
The Confidence Interval Is the Honest Number
Next to most composite scores, the report lists a range: "Full Scale IQ 98, 95% confidence interval 93 to 103." That range is the evaluator telling you the single number is an estimate. Testing happens on one morning, with one child, who may have slept badly. Re-tested next Tuesday, the same child would likely land somewhere inside that band.
If a district applies a cutoff and your child scored a 71 with a confidence interval of 67 to 76, the score does not settle the question by itself. Ask how the team weighed the interval, the classroom data, and the teacher input alongside the number.
What the Four Common Tests Measure
Four instruments show up in most K-12 reports.
The WISC-V, the fifth edition of the Wechsler Intelligence Scale for Children, is given to children ages 6 through 16. It produces five index scores: Verbal Comprehension, Visual Spatial, Fluid Reasoning, Working Memory, and Processing Speed. Younger children get the WPPSI instead. The Full Scale IQ combines those indexes, and it is the least useful number on the page when the indexes themselves vary widely. A child with a Verbal Comprehension of 118 and a Processing Speed of 79 has an average Full Scale IQ that describes no part of how they function. Many evaluators will say so directly in the narrative, and some will report a General Ability Index, which sets aside working memory and processing speed.
The WIAT-4 measures academic achievement: reading, math, written expression, and oral language. Woodcock-Johnson achievement batteries do the same job. These scores answer what your child has learned, not what they are capable of learning.
The Vineland-3 measures adaptive behavior through parent and teacher interviews, covering communication, daily living skills, socialization, and motor skills. This is the instrument that captures whether a child who tests well can manage a locker, a lunch line, and a bus schedule.
The BRIEF-2 is an executive function rating scale completed by parents, teachers, and sometimes the student. It reports on inhibition, emotional control, working memory, planning, and organization. Discrepancies between the parent form and the teacher form are data, not error. A child who holds it together all day and falls apart at 4 p.m. produces exactly that pattern.
Reading the Gaps, Not the Averages
The most advocacy value in the report is in the differences between scores.
Look for a cognitive index that sits well above or below the others. Look for achievement scores that trail cognitive scores in one specific area while matching in others. Look for a Vineland domain that lags everything else. Look for a BRIEF index elevated on both the parent and teacher forms. Each of these gaps points toward a specific need that a composite score averages away.
One caution on the older ability-achievement discrepancy model. Some parents arrive expecting a formula: IQ minus achievement equals a learning disability. IDEA 2004 removed that requirement, and states may not compel districts to use it. Districts now rely on response to intervention, patterns of strengths and weaknesses, or a combination. Ask which method your district uses before the meeting, and read the 13 IDEA disability categories so you know which criteria the team will be applying to those scores.
Turning Scores Into Goals You Can Ask For
A score is not a goal. The bridge between them is the present levels statement in the IEP, which is supposed to describe how the disability affects progress in the general curriculum, using the evaluation data.
Work backward from each gap you marked. A low processing speed index supports extended time, reduced problem sets, and advance copies of notes. A low working memory index supports written multi-step directions and checklists rather than verbal instruction chains. A specific reading subtest well below the others supports a named intervention with a measurable goal, such as decoding accuracy at a stated percentage across three consecutive probes. Bring the page number. "The report puts his phonological processing at the 4th percentile on page nine, and I want a goal that addresses decoding directly" moves a meeting in a way that "he struggles with reading" does not.
Request the full report several days before the eligibility meeting so you are not reading it cold in the room. Mark it up. If you disagree with the evaluation after reading it, federal regulation at 34 CFR 300.502 gives you the right to request an independent educational evaluation at public expense, and the district must either fund it or file for due process to defend its own. If the report raises more questions than you can work through alone, an educational advocate reads these documents for a living.
The numbers describe how your child performed on a set of tasks over a few mornings in a small room. They are useful, they are limited, and they belong to you as much as to the team that produced them. Ten days is enough time to walk in as the person in the room who has read page nine twice.
